Full Job Description - Read Carefully
The SDA Hospital wishes to invite eligible suppliers and service providers who are interested in doing business with the hospital to apply for pre-qualification towards the compilation of a supplier database for 2020 fiscal year.
Applicants will be pre-qualified in the following general area of supply:
- Supply of medicine, medical equipment, non-medical consumables
- Routine preventive maintenance service of incinerator and general plants, medical gas equipment.
- Air conditioners and refirigerator.
- Supply and servicing of office furniture, sanitary material, stationery and printing.
- Supply of computer and computer accessories.

- Supply of laboratory reagents.
- Supply of building materials and electrical items.
PRE-QUALIFICATION CRITERIA
- Application for pre-qualification would be assessed on the basis of registration using the criteria based on the indicators identified below.
- Brief introductory letter indicating location address and main contact of the person and the area of specification.
- Renewed Valid Business Registration Certificate
- Certificate to commence business.
- Pharmacy Council Registration Certificate 2019 (Evidence of payment of certificate must be presented for update if certificate is not ready).
- VAT Registration Certificate (Evidence of exemption if exempted)
- Valid SSNIT Clearance Certificate (if applicable).
